TERMS AND CONDITIONS FOR MOBILE OIL CHANGE:

I, the customer, authorize DNA Hybrid Solutions (“DNA”) to perform mobile oil-change services. I represent that I am the vehicle owner or have authority from the owner to authorize the requested services. I understand that an oil change is limited to the services specifically listed and does not constitute a complete inspection, diagnosis, or guarantee of the vehicle’s condition.

I understand that vehicles may have existing mechanical, electrical, structural, fluid, or other conditions that are unrelated to an oil change. DNA is not responsible for pre-existing conditions, defects, damage, wear, leaks, failures, or problems that were present before or that may present after DNA began performing the service. I understand that an oil change cannot prevent or correct unrelated mechanical failures, and DNA is not responsible for any issues that may occur.

I agree to inform DNA via email before service of any known, Oil leaks, Engine problems, Oil-pressure problems or warning lights, Overheating, Accident damage, Modified or altered components, Damaged drain plugs, oil pans, filter housings, or related components, Other conditions that could affect the requested service. Failure to disclose a known condition may affect DNA’s ability to safely perform the service.

DNA may rely on information supplied by the customer regarding the vehicle, including vehicle identification, mileage, oil type, oil capacity, previous repairs, modifications, and known problems. DNA is not responsible for problems resulting from inaccurate, incomplete, or misleading information supplied by the customer. If the customer supplies oil, filters, parts, or other materials, the customer is responsible for confirming that those materials are correct and suitable for the vehicle unless DNA has separately agreed in writing to verify them.

Unless otherwise specified on the invoice, DNA will use the oil and filter specified on the invoice. Customer-requested substitutions, customer-supplied parts, incorrect parts, defective parts, or parts that fail prematurely may result in additional problems or expenses. DNA does not warrant the quality or performance of customer-supplied parts or materials and is not responsible for the failure of any parts customer supplied or otherwise.

DNA will use the oil specification and quantity determined to be appropriate for the vehicle based on available vehicle information and applicable manufacturer specifications. If the vehicle has been modified, incorrectly identified, has inaccurate information, or has unusual requirements that were not disclosed, DNA cannot guarantee compatibility or performance.

I, the customer, understand that an oil change does not guarantee that an oil-pressure light, check-engine light, warning light, noise, vibration, leak, drivability issue, or other vehicle problem will disappear. DNA is not responsible for unrelated vehicle problems that appear before, during, or after the service. If DNA observes a condition that appears unsafe or requires additional repair, DNA may recommend that the vehicle not be driven until appropriately inspected or repaired, but the responsibility for those identifications are ultimately up to the owner and are their responsibility to recognize.

After service is completed, the customer assumes responsibility for operating the vehicle. DNA is not responsible for any damage that could occur during an oil change. I understand that continued driving after an oil-pressure warning, overheating, unusual engine noise, fluid leak, or other warning condition may cause additional damage. DNA strongly recommends that the customer immediately stop operating the vehicle and seek appropriate professional assistance if a warning light, leak, unusual noise, or other abnormal condition occurs after service.

I understand that mechanical components can fail unexpectedly and that an oil change cannot guarantee future vehicle reliability. DNA does not guarantee that the engine, vehicle, or any other component will continue operating without failure after the service.

Before operating the vehicle, the customer should review the invoice and notify DNA promptly of any suspected problem relating to the service. The customer agrees not to continue operating the vehicle if doing so could reasonably cause additional damage or create a safety hazard.

Mobile service is performed at the customer’s requested location. The customer is responsible for providing DNA with a reasonably safe and suitable work area, including sufficient space, reasonable access, and a surface appropriate for the requested service. DNA may refuse or discontinue service if conditions are unsafe, including unsafe traffic conditions, severe weather, unstable surfaces, inadequate access, hazardous materials, or other conditions that could reasonably endanger personnel or property.

The customer remains responsible for the vehicle and its contents while the vehicle is located at the service location. DNA is not responsible for personal property left inside the vehicle.

DNA may document the condition of the vehicle and the work performed with photographs, video, mileage records, service records, and other documentation for purposes of service records, quality control, dispute resolution, warranty administration, and legal compliance.

The customer acknowledges that used fluids and filters are hazardous materials and authorizes DNA to remove them from the customer’s property as part of the service.

Any additional work outside the parameters of an oil change will be documented in the invoice.

To the maximum extent permitted by applicable law, DNA’s liability arising from the services provided under this agreement shall be limited to the direct damages actually caused by DNA’s failure to properly perform the specifically authorized services. To the maximum extent permitted by law, DNA shall not be responsible for indirect, incidental, special, consequential, or economic losses arising from unrelated vehicle failures, pre-existing conditions, customer-supplied information or parts, or circumstances outside DNA’s reasonable control.

By agreeing to these terms and conditions, I acknowledge that: I have provided accurate vehicle information. I have disclosed known vehicle problems relevant to the service. I understand that an oil change is not a complete vehicle inspection. I understand that pre-existing or unrelated mechanical problems may occur before, during, or after service. I understand that an oil change does not guarantee future vehicle reliability. I understand that I should not operate the vehicle if an oil-pressure warning, serious leak, unusual engine noise, overheating, or other potentially dangerous condition occurs. I have had an opportunity to ask questions about the requested service via email. I authorize DNA Hybrid Solutions to perform the services listed.
